OPERA – COMMON CAUSES OF MAJOR LOSSES IN THE ONSHORE OGP INDUSTRIES

On 9^th June, Andy Goddard and Ron Jarvis presented to OPERA via ZOOM an update of their analysis of common causes of major losses in the onshore oil, gas & petrochemical industries. While the analysis was very interesting, it was disappointing to learn that the principal causes of large losses around the world remain as prevalent today as they always have been. Despite the emphasis that risk engineers have placed on piping inspection, isolation during maintenance and process hazard analyses during atypical operations, the message does not seem to have got through to the insureds and these issues continue to dominate the statistics. I guess we have to keep battling on…..
